Please bring your insurance cards and photo ID.  If you have any recent diagnostic tests including but not limited to x-rays, MRIs, ultrasounds or lab work please bring a copy.

On your first visit please allow 15-30 minutes to fill out our paperwork.  Please contact our office if you would like an online copy of the intake paperwork to be completed beforehand.

Following completion of your paperwork, our Doctors will review any ongoing issues or concerns.

After reviewing your intake, we will perform an examination to gain a better understanding of your condition.

Upon completion of the examination, we will begin treatment on the same day if warranted!
